Maven not spawning all 3 extra bosses with destructive play

While using scarab the new influencing scarab of interference i've noticed that sometimes maven will spawn only spawn 1 or 2 extra bosses while the whole map and mirage has been cleared.
It becomes really obvious with the scarab since each extra boss she spawns also spawns a guardian.
