Memory Tears not spawning at all?

Hey I had an issue that was described in previous post that Item didnt drop from Memory tear , but i been doing t16 maps with all voidstones unlocked and also with extra passive points allocated for said memory tears and i didnt find a single one I would say i did solid 50+ maps or maybe more since they dont seem that rare looking at trade economy but im ssf trying to farm an item and i cant because there is no way for me to access incantation of fear boss the only time i killed him was a quest version and i was actively trying to get to him since and i wasnt able to.

I don't really have any solid evidence for that or screenshot to provide but i feel like im bugged out of ever getting memory tear in my map.
