Unexpected disconnection occured on entering the map again after dying

Well, as the title states, I was doing burial chamber, i died during boss fight, i tried entering again, i got disconnected unexpectedly,
I re logged in, got in again, got disconnected again after loading screen.
Happens everytime in the specific map which was open when i did /bug and portal gets used up

Okay, so I opened another burial chambers (man's gotta complete bonus objectives)

I put portal outside boss room, i died to boss again, went in again, boom disconnected again on a new instance, this time, on vulkan, so its reproducible for me on Vulkan and on DirectX 12
