Vruun always spawns invisible

The breach boss Vruun always spawns invisible with only his hammer visible. You have to reenter the map to see him. It does not matter if the Unstable Breach was opened in a normal map or inside a mirage.

In addition the visual effects of the Unstable breach are missing frequently aswell, with no glow around the hand or the actual breach area being visible.
