Uhtred's Omen - Bug

I slotted Uhtred's Omen into Time of Need first with no other support gems slotted and it disappeared and I believe it removed one of the slots from the gem. This is the second time this has happened with that specific support gem.

Please feel free to review my logs if possible. Second time happened around 11:45ish EST 3/7.

EDIT: Still unsure if it removed a slot from the gem (can't recall if I had 4 or 5 slots), but the Uhtred's Omen appeared to be slotted into one of my empty mace skills. Possible user error.
