Divine Vessels disappear from map device after map completion

I have attempted twice to use a divine vessel to upgrade Soul of Lunaris. The first map worked as intended. The second map, Iceberg T7, deletes my Divine Vessel on map completion. Has happened twice on Iceberg.
As far as i can tell, it shouldn't be roll-back related

Same, I wasted 3 Divine Vessel on Iceberg and got nothing
Lost 2 to Iceberg here myself. Tried everything I could think of. Checked inventory, map device, stash, and even the pantheon itself in case it automatically claimed the reward. Nothing. :(
Same thing happened to me on an iceberg T7. The vessel is used, but it disappears and you can't claim the pantheon.
Iceberg did the same. Ate my vessel ;(
Same, the boss of the map doesn't match the name in Pantheon.
Same thing happened to me, Iceberg is eating divine vessels
Iceberg randomly has one of three bosses, and only one of them has a soul you can capture.

A pantheon unlock being locked behind a map that has a 2/3 chance to just delete your divine vessel and waste your time is bad design.

GGG forget what happened in 3.15 which led to a fix in 3.15.1

It just devs either a gold fish, or new hire doing something oops on a game that have a lot of legacy code
