They don't only drop from the ubers, even the quest & normal ones can drop them.
I think ubers can drop the hidden ascendancy point of the class though

On the drop table listed here it says for the regular encounter, it doesn't drop as "encounter specific", but can drop as an additional item, listing 5%.

I think the quest item table might be the same as this, except without possibility of modifiers?

Looks to confirm what imanubcake is saying about the hidden node as well.
