BUG - Activated Omen of Amelioration ignored on Death

Hi,

the Omen of Amelioration just bugged out for me.

I had a stack of 2 in my inventory activated, died, 1 was used, the other still being shown as activated (i checked it), died again, this time it wasn't used at all, it is still shown as activated in my inventory after i died?
