Add a Support Gem for Skills with Instance Limits.

When a skill reaches its instance limit, such as with Living Bomb, the default behavior is to remove the oldest instance. This support gem would alter that default behavior by preventing the creation of new skill instances and instead refreshing the duration of the oldest one.
