Sinking Spire Tower map not completable after upper level entrance disappeared

Sinking Spire map has entrances in the corners until you reach the roof. After clearing a double overlapping breach, abyss, and vaal ruins circle, the entrance to the next level completely disappeared from the map and I cannot progress. Leaving and coming back does not bring back the entrance
