How would u improve my build ?

Kinda new so explain in simple terms please, I don't know how any of this works

I'm tryna go for a mix of i do half the damage my wolves and skellies do half the damage; I don't really have anything to complain about in terms of damage, stuff dies, but I myself also die so easily, and I don't know how to fix that really

Stuck at around T7-T8 maps and can't seem to level up anymore (cause of the dying)

Is this just skill issue cause i'm not meant to get hit with all my minions or is there something else to do here?

Its very simple: Your build has bad survivability. You may want to read/watch some resources on PoE basics. Zizaran supposedly has some useful/good vids about that. Something like "PoE University" or so.

0. Unless you really know what you are doing (clearly not the case) invest in Life not in Energy Shield.
1. Use at least one out of Grace/Determination. Discipline however is not very useful on such a build. Try to have some %evasion or %armour as well - the one befitting your Aura. Your tree looks like Determination would be the better choice, also get "Combat Stamina" then.
2. Better gear: Most rare gear pieces should have a strong life roll on them.
3. Better Passive Tree: You need to invest in %life on the tree. Improve efficiency.
4. Get a life flask.
5. Get some more Chaos resistance.
Etc etc etc. See above: Learn the basics with a guide somewhere.

No, its a build issue. You "should" build so that you dont randomly die even if you mess up a bit.
