Bug Report ID: #2017620121 Getting respawned and stuck outside the playable map area after death

Doing a T11 four mod Flotsam map with abyss and ritual tablets.
Abyss had a depths area i died in, i continued the map did a ritual and died in that and it respawned me outside the walkable map area.
Playing a druid shaman and was shapeshifted to a bear when i died and it is my weapon slot 1 form.

I can open a portal and leave to the hideout but i cant respawn at checkpoint so the map is bricked.

This just happened to me after dying in a T15 Flotsam map
