Yes; you would need to repurchase both the game & any MTX you own.
Having spent thousands of euros on Path of Exile over the years, I will not be further supporting it financially until and unless GGG resumes offering Technical Support.

As long as your account is linked to both you would still be able to continue your characters and progress.
