Improve consistency of map icons and quest notation for permanent upgrades

The map icons with + symbols are a great way to know what I need to go back for. I don't think it's 100% consistent though. Just now we did the Molten Vault and there was no indication that it was going to be important, but it unlocks the reforge bench. We looked on the quest panel and there wasn't anything there.
