Sometimes Strongboxes drop no loot at all (also Vaal encounter in maps)

Just had two strongboxes in a row, identified and regaled them, killed the mobs and got no loot from the box itself. And I mean "pressing alt no loot", not a single item dropped from the boxes (loot from mobs did drop).

Also the Vaal Temple encounter in areas used to regularly drop an ex when finishing it during campaign, but as soon as I hit maps it stopped dropping anything. Is that intended?
