Shred Ice Fragments Randomly Stop Working

Whether it be a zone switch or using a portal, my shred will not generate ice fragments on frozen targets until I return to character select and load back into the game. Wondering if anyone else is having this problem, I don't believe it is visual, because there is no damage afterwards. It also seems to be affecting the ice fragments that should be coming from my frozen spite support gem connected to herald of ice.

Currently on Dreadnought and ice fragments are not spawning for me, either.

Unequipping/Re-equipping the Talisman fixes this, though.
