Uneasy Roost - Core Map Issues

Just ran the Uneasy Roost Core Map and found two issues:

1) Rares did not seem to spawn Malachai (or any of the trio either) despite the 20% chance
- Killed 39 Rares total and 6 Unique Monsters (1 each of Kosis, Omniphobia, Shavronne, Maligaro, Doedre, Malachai)
- With 20% chance it should have been around 8 extra Malachai encounters or around 14 Unique kills total)

2) Malachai was not ghosted in the boss fight
- Shavronne, Maligaro and Doedre were
