HC SSF Ruthless Breach encounters quest item not dropping second time after first one was lost

Hello there,
I have problem that I got Severed limb dropped in Act 6 but before I was able to deliver it to Ailith I died. So I lost this limb and I already had over 100 breach hive encounters in act 6+ since then. I have never got drop of this Severed limb again while I was able to collect most of the other Breach quest items. Then I realised that it might drop only once even though it wasnt delivered to Ailith and it is probably a bug.
So it is a bug or I just have got really bad RNG.
Anybody with same problem?
