Trial of Corruption Bugged and Can't Progress

I died during the trial of corruption. After respawning the door to access is still open and removing the ascendancy quest item from the alter and reinserting it does not refresh the instance to let me launch the trial again. I've respawned again and still nothing, quit the game entirely and relaunched and I keep getting the same in game message that says - The operation could not be completed. Cannot connect to instance. The subarea no longer exists.

Has anyone had this same thing happen? If so, how did you get around it?
