Flasks do not work at all and consume all charges on use, 1 ritual not working

Mods:
Area has Patches of Shocked Ground
All Monster Damage from hits always ignites
Monsters cannot be taunted
Area is inhibated by Lunaris Fanatics

Map Mechanics:
Gwennen, Harvest, Ritual, Breach (the one that expands and summoned the breachlord)

No scarabs used, 2 voidstones

Issues noted:
Activating any flask consumes all charges on use and shows no buffs (My movespeed flask normally uses 30 our of 60 charges). No movespeed increase observed when activating the flask. No buff appears in the top UI.

One of the ritual altars does not unlock after killing all monsters in the area. Was able to complete 3/4

The issue only happened on this map - did not repro on other maps.

My natural assumption would be Searing Exarch influence, with a particular altar downside, buffed by the "Wrath of the Cosmos" Atlas keystone passive.

The particular downside grants 20-40% increased charges used, and 60-80% reduced flask duration.
Wrath of the Cosmos has the effect "Eldritch Altars Influenced by the Searing Exarch which have an additional Upside have 100% increased Effect of Downsides". If the Altar had 2 upsides, the effect would then be increased to 40-80% increased charges used, and 120-160% reduced duration.

All of your flasks use at least 30% of the flask's total flask storage per use normally, so at 66% increased charges used, the flasks would not be able to use more than one full use at a time.

With over 100% reduced duration, you would need increased flask duration (from sources other than the flask themselves) so see even a brief uptime.

Ritual Altars do not unlock unless you kill enough monsters in the vicinity of the Altar. If monsters that spawn near the Altar are lured away and killed elsewhere, you can't activate the Ritual Altar.
I had no Exarch altars active, it was apparent from the moment I spawned into the map.

Yes I'm aware how ritual altars work, there were no monsters nearby.
Exarch altars are unfortunately the only thing I know of that as well that can "randomly" cause that exact same condition where you can vastly increase the charges used and reduce the duration to near nil, and will fix itself the next map.

Not much else you can do from here except check the altar mods if you see it happen again or hope GGG takes a peek at the bug report amongst all the others.

The Ritual bug has been around for awhile now. You can reduce the map monsters to 0 and it still will not unluck the Ritual unfortunately. I recall some theories about what could cause it, but it wasn't entirely consistent from the people reporting it.

Specific monsters in the Ritual, Mavened bosses, or just some random desync and the game didn't correctly capture the last mob dying so never unlocks.. those were just a few of the possible theories. Haven't seen it yet this league, but probably in total around a dozen times of unclickable altars with no monsters remaining.
