Unable to play game due to loading screen issues

I am a relatively new player, I have maybe 20hrs in the game. Since the first time playing, I’ve had constant issues with getting “unexpected disconnects” during loading screens for various areas. In the past, this has been annoying because it happens at least half the time I load into a new area, but because the loading only took 10-20s it wasn’t a huge deal to just reconnect until it worked. I’ve of course tried every fix known to man, such as
updating drivers, changing renders, changing all sorts of graphics and game options, disabling firewall, clearing cache files, even installing the standalone client(I normally use steam, standalone client performance was even worse).

Now, I just finished the Act 2 final boss, and am trying to get back to the caravan to finish the act, however, the disconnects are horrible, it takes dozens of tries just to load to the boss area, and then trying to portal to the caravan gets stuck on an infinite loading screen. I’ve restarted my pc and done the above changes several times with no results. I literally cannot progress the game at this point and it sucks.
