Omen of Abyssal Echoes disappeared unused

Yesterday in my hideout, I used an Omen of Putrefaction on a Quarterstaff.

Then I moved a 3-stack of Omens of Abyssal Echoes into my inventory and activated it (red border was showing).

At the Well of Souls, after I had revealed all the staff's affixes all my Omens of Abyssal Echoes were gone - and I did not even use them for a reroll (i.e. I did not click on the "recyle" button with the two circular arrows).

The chat just showed "Omen of Abyssal Echoes was consumed." or something along the lines.

It would be nice if this bug could be fixed. Or maybe add a confirmation dialog.

Omen is used before reroll - it gives you reroll option.
