Life flask not refilling.

We all go through times clearing maps when your E.S. then life drops to near zero. The issue I have been having is that my charges (7), E.S. recharge to full, not my life flask, may refill to 1/4 to a third.
I am playing Abyssal Lich with full life, this started a few days ago. An issue when you are in a tight spot, the you have is not where it should be. The only time it refills to full is upon a death.
