MOM+Zealot's Oath+Controlled Metamorphosis did't work as expected/they should be

okay，My bloodmage has MOM、Zealot's Oath(Using Controlled Metamorphosis),I died a lot with my mana Pool full, there was a time I have the mana flask that removes life, I thought it was this flask which cause the fluquent death, but it turns out it's not the flask, after I change my flask to normal one, I still die with mana full, couple times a day.

once I thought it could be some mod I steal from rare mobs with headhunter,but today I was fighting with a boss for 2+ minutes and then get one-shot by the boss, and still mana pool is full.

so I think there must be some bug around the combination.
