Map bugged/no citadels maybe related?

Ive spend quite some time mapping on my character, ive found no citadels and sometimes encounter issues where the connecting lines between maps shown are incorrect. Maybe this is related to each other, but im now quite certain about that.

https://imgur.com/hXuEHPR here you can see on the player icon map that i failed it. however it still connects to the map above that i can run without actually completing it. Before I failed the map the connecting line was different as well where the unlocked map above the player icon was connected to the completed one left of the player icon.
