Inscribed Ultimatum/Djinn Barya bug

The Chaos temple mechanism ate the item without me using it.
Inserted the Inscribed Ultimatum in the mechanism at the Chaos temple, doors opened (I didn't enter) however when I randomly opened the mechanism again, the item was gone and the doors closed.


Relics and item were removed
After dying in the second floor of the Sekhamas trial, I did not get my relics and Djinn Barya back. Not sure if this is normal, however I've read in forums that you get the stuff back upon not completing the trial.
