Shrines not spawning with buffs, boss hp irregular, amelioration omen bug

1) Shrines are not appearing in maps compared to patch 0.3.0xx . Some deadly bosses only spawn 1 shrine even with tablet "1 additional shrine" - mod. I have the atlas skill point of +1 shrine in stonger boss maps too. Maps are tight corridor types and tight ledge types.
Shrines altogether do not spawn in relation to taking atlas points with shrines. Patches before had much more shrines in maps. Now, even with the tablet mod "+1 additional shrine" together with ALL shrine buffs from atlas that increase chance AND using one or two tablets together with the +1 tablet that has increased chance of spawning shrines, do not yield results.
I have usually 500% - 700% increased chance of shrines with the additional from tablet and increased chance from other tablets and using additional from boss atlas skill tree. Mods show up in right top screen when entering map, still mostly I have only 1 shrine in maps. Maybe confliction of using several irradiated tablets together? they still show the mods in the map though.

All things considered this does not represent the worth of usage of the atlas points in my opinion anymore.

2) Some bosses, without any +% hp mods to them, are far more stronger than for example deadly map bosses, even with the deadly ones having more map mods. I'm not opposed to strong boss battles, but having 20million hp with a map boss that takes reduced damage is sometimes a bit much.

3) Omen of amelioration does not activate when I die. I'm monk lvl 97. I reset my level experience by trying this. Omens did not activate when I die, instead 2 of them just disappeared randomly when I entered or came out of map (I died 5 times). So it did not use all of them, and when I died I of course lost the full amount of experience. I did die 3 of these times on purpose to a boss. It was delirious map with abyssal mod on the map and 6 mods and 3 tablets.

Frustrating but I only need to be much more considerate towards mapping now.

Instances with omens of amelioration were 3 different maps. And they still did not 'pop' inside the map or save the experience points.

Can confirm that something is definitely wrong with shrines spawning since 0.3.1
