Simulacrum 16/15 waves bug

I did a 50 simulacrum run. I have all resistances capped at past 100, run tier 14 maps easily and have to be more cautious running T15 maps. By wave 9 I was getting swarmed within 15 seconds and dying, and after a few deaths close to the portal, the portal would be surrounded by mobs upon respawning. I'd be blocked by 50 mobs and without being about to move or being able to cast a single skill before insta-dying. All together throughout the entire run, idk how many times I died and respawned, probably 100+ times.
Eventually within each following wave, I managed to position the mods upon my death in such a way to create a gap through which I would dodge roll out. Eventually I cleared the 15th wave.
Nothing happened, no pop up, no loot drop. I exited the instance and came back. Upon returning to the instance I was greeted by the same challenge banner that you click to start a new wave. However this time it said "Wave 16/15 : Click to start the next wave". When I click on it, nothing happens, I click on the floating dude, nothing happens and the last boss that I killed is still groveling while his shadow is standing still in the middle of the arena.
