Omen of amelioration selected but not consumed

I just died at l95, made around 10% of progress and had the omen of amelioration activated. There is a stack. It didn't consume and basically wrecked me.

Two things i noticed:

Sometimes when you have a stack of omens, it is selected, red frame is showing. Yet, no dice, it does not activate. So you have to reactivate it every time, which is a pain since the indicator says i am safe. Lost all my progress today.

Second: Sometimes it just does not activate, at all. I mean, i have only one omen active, i die and i should lose 2-3%, but there, my progress is gone.

This should be ficed and omens should probably all work together. So basically a Resurgence and Amelioration should work together. Right now, it is one amelioration, and it kind of triggers 80-90% of the time.

I guess there is some easy fix to this I hope.
