Not a bug. You clearly had added deli orbs aka "liquid emotions" to the map. If a map is already delirious, the mirror won't spawn.

also just encountered a map missing normal monsters (overrun by the abyssal) |

Finished clearing the 2nd map I posted, only abyss and coalesced monsters were present.

also IMHO, the audio stacking/clipping is worse post patch, at least on my system
