Ascendancy Points Not Awarded After Trial of Chaos 4 Rooms

Hi

After finishing trial of chaos 4 rooms I teleported out without collecting ascendancy points. Re did the trial and still cannot access the next 2 ascendancy points despite refunding passives and using correct quest item. quest indicator is stuck at complete trialmasters challenege please help!!
