Getting teleported beside monsters

I have on multiple occasions been teleported near monsters and monster packs, as well as been teleported near a boss, all while using a bow.

This is especially prevalent inside a breach where I am on one part of the map and then I get teleported to another part of the map where there are monsters or monster packs without me moving or what.

There have also been a number of occasions where I'm trying to attack the boss from a distance, but I get teleported right beside it, and I am hit with the slam. This has happened a number of times with the monkey boss in the ice mountains map.

This has been happening to me all damn day with headhunter. Porting straight into massive packs and getting surrounded where I cant roll out or into boss skills. Havent seen any rare mobs randomly teleporting to my location so im not sure what rare buff would be causing this... highly annoying.

I feel like it gets worse inside breaches. Wind Dancer has saved me multiple times when this happens with normal mobs but during a boss fight with one shot slams, I get absolutely wrecked.
