Act 3: Treasure of Utzaal Quest Item Bug

Hammer of Kamasa quest item does not drop. I've recently made a new character and was at the Molten Vault. I've beaten the boss a couple times and the quest item hasn't dropped.

You only get one Reforge Bench for all characters in one league.
So for any other character the quest item doesn't drop again. But now since 0.3.1 (0.3.1 Patch Notes) the quest description should be improved: " |
