Simulacrum bugged - no book after wave 15

Can't complete simulacrum. No drops after wave 15. No book for atlas passives. This has happened twice. Used 50 shards each time. Can't advance atlas tree. Please fix.

The second time, I logged out, came back in the instance. There was a 16/15 display but it couldn't be clicked.
