Omen is broken

Amelioration seems broken, several times it will not kick in. Its suppose to take away 75% of your death penalty per map. It's becoming very frustrating. Anyone else experiencing this crapola?

Only 1 Omen can be used per map instance. If you already used one in the map, all future deaths in the same map will not activate an Omen.
This effect is across all omens. If you have any other omens that trigger prior to death, then the on death omen will not be available for use as well. “Freedom is what we do with what is done to us.”

Yeah, are you using or picking up any other omens in the maps that might be proccing first? I think the omens give a message to the player chat when they proc, so maybe check that if it happens again. Also might be worth hiding other cheap omens on your loot filter. In POE2 I saw you have to activate them in the inventory, that might be something they think about adding. I've used the omen of Amelioration a fair bit myself and not run into any difficulties yet.
