Flask Charges Disappear

My health flask charges are disappearing. Sometimes when I am hit sometimes just randomly decides to disappear. They also don't recharge despite flask giving rechargability. Can only refill at the Well. Started after most recent hotfix. The difficulty has increased immensely and feel like I can't play this character anymore.
