Omens not saving my life

The omens to replenish shield health and charms are not working 100% of the time. Also the 75% doesn’t work half the time. Also the omen to rejuvenate health and shield gets used when I portal into the well. If it’s activated and I go to the well it uses one and text prompts its use. Very annoying. 100 seems so far away at 96.
