Sacrificial Heart bug with party play

I have noticed a bug where the knife for the sacrifice doesn't spawn after certain conditions are met. Below is the steps to reproduce the issue.


I received the Sacrificial Heart drop in Utzaal when playing solo and then joined my friends for few quest in earlier maps.

The next day my friends completed the sacrifice in Aggorat before I joined the map and put up a TP for me to also complete the sacrifice.

I got the animation for putting the heart on the alter but couldn't find the knife for the sacrifice.

I assumed the knife spawns after putting the heart on the alter not realising the knife was meant to have spawned before putting the heart on the alter.

I have run the Aggorat map fresh 2-3 times and every time the alter has the heart on it but no knife spawning.

I have tried farming the heart again on Utzaal few times to see if the quest resets but to no avail.

I have noticed other similar bugs with party play but this is the only one I have come across where the reward affect the character progression
