Blood Sacrifice skill point reward bugged for me?

I'm trying to track down the last of my passive skill point awards and I think Blood Sacrifice is the remaining one. On my map it shows the +?, when I try to go to the altar I see a heart on it but can't interact. When I try to go to Utzaal and get the heart it doesn't seem to drop for me. Have tried a few things but can't seem to get it to 'kick-in'....ideas??? Maybe the map graphic is bugged and I already got these (and will continue to check) but want to be sure it's not a known issue..
