Disappearing loot from Chests

I've noticed Sometimes loot drops then is gone. And even after turning off filter the loot is not there. Ivee had this happen in Alva temple, and Vaal temple Maps. It has happened quite a lot.

Did you press Z to disable item labels?

I don't have Z bound, are you referring to toggle highlighting? Yes and no it doesn't change anything. Same with highlight items and objects.

The key/button bound to "Toggle Highlighting" will toggle the "Always Highlight" option on/off.

The behavior you describe in the OP is the game's expected behavior with "Always Highlight" off:
Some items will always be displayed
Other items (if not hidden by filter) will be displayed for a few seconds, then the label will be hidden
Labels for portals, Vaal side areas, chests, rocks, doors, etc. will only display while the object is moused over

The key/button bound to "Highlight items and objects" will, while held, show the following behavior:
If "Always Highlight" is off, will display labels in the same way "Always Highlight" does when on
If "Always Highlight" is on, will also display labels for items hidden by the item filter.

either way pressing the toggle highlight button does not bring the items back.

I've even had this happen in inscribed ultimatum. Where i go and turn my filter off, toggle highlight off and on again. nothing, holding alt also doesn't show make the item come back.
