Persistent Connection Error: "Disconnected. Failed to connect to instance"

Dear GGG,

I am writing to report a severe and game-breaking bug that has persisted for the last three days. The issue consistently prevents me from creating new map or boss instances after exactly two successful attempts, rendering core endgame content inaccessible.

1. The Core Problem & Reproducible Pattern:
When I create/open a new map or boss instance (e.g., placing a map in the device, opening an Uber Exarch arena), I often receive the error "Disconnected. Failed to connect to instance" upon trying to enter it. This forcefully disconnects me from the game.
Crucially, I have identified a 100% reproducible pattern related to INSTANCE CREATION:
1. After performing a major reset (clearing game cache, reinstalling the game completely), I can successfully create and enter exactly TWO (2) new map or boss instances.
2. On the third attempt to CREATE/OPEN a new instance (e.g., placing the 3rd map in the device, opening the Uber Exarch arena for the 3rd time), the "Disconnected. Failed to connect to instance" error occurs when I try to enter it. This pattern repeats without fail.
This has been observed consistently with:
Various map tiers (T16, T17)
The Uber Exarch encounter (most reliable trigger: 2 runs possible, 3rd fails on instance creation/entry).
Corollary Issue (Last Portal): Other players can sometimes enter instances I created before the last portal is consumed. However, if it's the last portal remaining in a map device, other players report being completely unable to enter, receiving errors like "portal is closed from the other side" or similar. This suggests the instance creation/connection mechanism fails catastrophically after the initial successful uses.

2. Troubleshooting Steps Already Performed (No Success):
I have exhausted numerous standard solutions, all without resolving the issue:
Restarting the game client repeatedly.
Switching between different game realms/gateways.
Using various VPN connections (different locations/providers).
Manually deleting the game's cache/content.ggpk file.
Using Steam's "Verify Integrity of Game Files" feature.
Completely uninstalling the game via Steam.
Manually deleting all remaining game folders and cache after uninstall.
Performing a clean reinstall of the game.
The Result: After every clean reinstall or cache purge, the pattern immediately returns: I can create and enter exactly two (2) new instances. The third attempt to create/open a new instance fails with the disconnect error.

3. Impact:
This bug completely blocks my ability to engage with mapping and bossing – the core endgame loop. It is incredibly frustrating and makes the game unplayable for me. The loss of maps, fragments (like Exarch invitations), and potential rewards is significant. The perfectly reproducible pattern (2 creations allowed, 3rd fails) points to a deep-seated issue.

4. Request:
Please investigate this critical bug urgently. The reproducible pattern (exactly 2 successful instance creations followed by guaranteed failure on the 3rd attempt) strongly suggests:
A bug in the instance creation/management system specific to my account/session state after initial actions (like reinstall).
A server-side resource limit or leak tied to my account triggering after two instance creations.
Corruption specifically related to the mechanism that allocates new instances after the first two.
