Breach doesn't close after using special tablet

I used a tablet that keeps the breaches open until all breach enemies are defeated. The problem now is that I defeated all of the monsters but the breach won't close. That means no rewards.

One thing to note is that I died during the breach, but because the breach stays open, I started at the checkpoint and finished it. I'm guessing that something about dying during a breach that doesn't close will make it glitch.

I've restarted the game, gone back to the hideout, checked every corner of the breach circle... everything. Nothing will fix the glitch
