Omen of Amelioration Doesn't Trigger

I don't know why but sometimes this omen doesn't trigger. I die and the omen doesn't get used.
Not sure why this happens as it has been happening in all different scenarios but I think this bugs out nearly half of the time. Quite frustrating as I lose a lot of XP at high levels. Cheers. Last bumped on Jun 19, 2025, 5:23:30 PM

I have run into this too. I get it 100% of the time when there are no revives left on the map.

Make sure u dont have others omens like flask ones or else activ in the same time. I thought i had this bug too until i realised u cant combine em
