Game CRASH when entering/exiting DEMON FORM

Hello,

The game keeps disconnecting or crashing when trying to use demon form. It first happened when I switched my ascendancy points to use demon form.

Then it repeatedly happened whenever the demon form skill was used or exited.
If activated, the game disconnects but the character stays in demon form>|

After trying to cancel the form, or if life runs out, game crashes again.

This has happened over and over again every single time.

Tried changing servers and restarting steam and computer ect...
Didn't have this issue last week.
