unable to fire (major issue) and fire get stuck (less of an issue)

Hi,
sadly i dont have a video to show as my recording was off.

the major issue:
this happened to me many times in game, i think normally after clicking a shrine/a strongbox - where i am just unable to fire at all (normally i play range), sometimes i might be able to gain the ability to fire again if i try using a new skill. relogging normally helps.

minor issue:
this happens to me a lot when using 2 different crossbow bolts, lets say im spamming bolt A (bolt is already selected, i attack with right click), i release right click, choose bolt B and right click to attack - a lot of the time, nothing happens, i will prob need to release right click then reclick it.
