3rd time My omen of Amelioration isn't working

Just died again, and yes I click and highlight the omen so it's on to be used, and the omen is still there and i lost an entire bar of XP again. This is the 3rd time I know of where the omen didn't get used and at least twice I also lost the XP with the use not happening. What is going on or am I missing something. This XP loss at level 91 is crushing my desire to keep going at this point cause it keeps happening.

EDIT- it legit JUST happened again, Cmon man. That's 3 bars of XP gone in a row. went from halfway to 92 now needing 80% again. ughhh

Are you using other Omens in the area or are they kept in the Titan extra inventory?

there's at least one post on this issue daily and the answer is always the same. only one omen can be triggered from combat within the same instance as it tells you on the omens. if i had to guess, you're either:

- using multiple different omens and a different one triggers first making amelioration obsolete in that map
- you've already triggered amelioration in that map and you put on a second one to finish the map and the second one didn't work.

if you use omens properly, they work every time (at least for me and almost everyone else)

I am not using anything else but the amelioration. I know you cant use more than one and I don't even have more than one in my inventory let alone another highlighted. It happened back to back times right , the 2nd one immediately after I posted this here.

back to back as in trying to use 2 before finishing the same map or it failed back to back in 2 different maps?

I had one highlighted in a juiced map and I died. I went to teleport back and I noticed it was still in my inventory and still highlighted, and that is when I noticed the XP gone. I went back to hideout, took it out, put it back in, highlighted it a few times off and on to make sure, and then went to fight the boss to finish the map. He one shot me and again, the highlighted omen is still there in my empty inventory at this point and another full XP bar is gone. so 2 times on one map.

No other omens at all , and the 2nd time just now I had gone back to hideout to clear inventory except for currency and the omen. Went back for the boss to finsh the map , got one shot, and boom omen still highlighted and there in inventory and another full XP bar is gone.

It was the same omen and it happened two different times on the same map. 1st time to mobs during a breach, and this last time I went to hideout , emptied my stuff except for omen and currency, lit it up , got one shot by the boss and the omen is still there lit up but whole bar gone since it wasn't used.
