unable to attack, when activating Delirium and chests

I have experienced several times, when i activate delirium, or click a strongbox, i cant attack, and die .
this happens more than 90% of the times.
Please look in to it, its annoying as hell.
Im a lvl 93 witch, with a Rake bleed, lightning spear.
