Map Quest Icons - Some dont complete

Not sure if inteded or not... but i have hit several map node quests repeatedly, and the icons dont even grey out with an x for completion.

Most notably are the camps in act3, agnarok witch.

Also, i noticed that you can seemingly bug out some others and they will not be ever completable if you do. For eg: the elemental rings quest in act2. If you open all the containers, but DONT loot the items, later, when you return, you can no longer open the containers, and you will never be able to complete it.

Just small things that cause aggravation to the ocd.
