ExpeditionCrash, Offhand Skills Not Worked

Expedition Crash in Expedition Logbook instance when a boss's tomb spawned near the first portal when player enter the map (Most of the case, but if the boss spawned far away it doesn't crash)

Also skill that in off-hand sometimes bugged and not worked (Mana consumed, but skill did not go off), mostly happenned to Curse Skills.
On top of that Blink sometimes also didn't work when use the keybind (space) the charcter is not rolling or use the Blink (This blink also only checked on 2nd weapon slot). I have to take the Gem out of the slot and put it back to make it work.
