Omen not working..

Both my Omen of Amelioration and Omen of Resurgence didn't work and caused me to have to grind for another 20 mins to gain back the xp I lost. Please fix lol

This happens to me 3 out of 5 times I die. Omen is activated but doesn't always get triggered. I die... check inventory and It's still there & still activated.
